MJC will email the application form to the applicant’s email address after he/she “submit” the form. Please ensure that your email address and other contact information are up to date and correct.

Don’t forget to attach together the required documents that are mentioned in General information: Session 9 Application Guidelines, No. (2) Application Documents, page 5.

For No. 7. Health Certificate, you don’t need to submit now.

All these documents except for the declaration should be typed, printed out and signed by the applicant and the head of the relevant department / division of the organization in charge on all pages. Handwritten forms will NOT be accepted.
